It is the question I hear most often in my consultation room on Harley Street: once I have had my dermal fillers, how long will they actually last? The honest answer is that longevity varies more than most patients expect. It depends on the product used, the area treated, your own metabolism and how the filler was placed. In this guide I want to give you realistic timelines rather than marketing promises, so you can plan your treatment and your budget sensibly. Most of the fillers we use are hyaluronic acid based, which behave in a predictable way and, reassuringly, can be adjusted or dissolved if needed.

So how long do dermal fillers typically last?

For most hyaluronic acid fillers, results last somewhere between six and eighteen months. That is a wide range, and it exists for good reason. A soft, spreadable filler placed in the lips tends to break down faster than a firmer, more structured product placed deep on the cheekbone or jaw. Movement is the main factor. Areas that are constantly active, such as the lips and around the mouth, work the filler harder and speed up its natural breakdown. Static areas like the cheekbones sit relatively still, so the product persists. The type of gel matters too. Thicker, more cross-linked hyaluronic acid resists breakdown, which is why it is chosen for deep structural support, while lighter gels used for fine lines or lip hydration fade sooner. Blood supply plays a part as well, since a richly vascular area clears the product more efficiently. What affects how quickly my body breaks down filler?

Individual biology counts for a great deal. Patients with faster metabolisms, and those who exercise intensely and regularly, often find their results fade a little sooner. Smoking, significant sun exposure and general skin ageing can all shorten the visible lifespan of a result. Age and hormonal changes influence how your tissues hold volume too. There is nothing you have done wrong if your filler seems to go quicker than a friend’s; we are all different. What happens as the filler wears off?

To an extent, yes. Protecting your skin from sun with a daily SPF, staying well hydrated, not smoking and maintaining a stable weight all help your results hold their shape. Booking maintenance appointments before the filler has fully dissipated usually gives a more even, longer-lasting outcome than starting from scratch each time. Equally important is the skill of placement; correct depth, the right product for the area and a conservative, well-planned approach all contribute to results that both look natural and endure. I would always caution against chasing longevity by over-filling, as that rarely looks good and is not the aim of considered aesthetic work.

Some patients feel results seem to hold slightly longer with repeat treatments, and there is a view that maintaining volume can reduce how much product is needed later. Evidence on this is limited and individual, so I would not promise it. Longevity still depends mainly on the area, product and your own biology.

Is there any downtime after dermal fillers?

Most people return to normal activities quickly. You may have some swelling, redness or bruising for a few days, particularly with lip treatment. I advise avoiding strenuous exercise, alcohol and excessive heat for around twenty-four to forty-eight hours. Full settling of the result typically takes about two weeks, so avoid judging it too early.
