Why Is My Scalp Itchy After a Hair Transplant?

Itching is one of the most common complaints in the first weeks after a hair transplant, and one of the most difficult to ignore. In the great majority of cases it is a sign that things are healing rather than a problem.

Why does it itch?

Several things happen at once. Wounds itch as they heal, which is simply part of the process. Crusts drying around each graft pull slightly on the skin. Nerve endings disturbed during surgery send odd signals as they recover, often felt as prickling or itching. The scalp is also drier than usual, particularly if you are washing differently and avoiding your normal products.

Itching often peaks in the second week, around the time crusts are lifting, and settles as the skin normalises.

Why you must not scratch

In the first fortnight, scratching can dislodge grafts that are not yet fully secure, and once a follicle is out it does not go back. Later, scratching risks damaging healing skin, introducing infection and leaving marks.

If you scratch in your sleep, that is worth mentioning to us; keeping nails short helps, as does sleeping with the head elevated.

What helps safely

Wash exactly as instructed, since gentle regular washing removes the crusts that are driving much of the itch. Use any moisturising spray or lotion we have given you. Keep the scalp cool and out of direct sun and heat, avoid sweating heavily in the early weeks, and keep hydrated.

Do not apply oils, home remedies, anti-itch creams, steroid creams or anything else without asking us first. The scalp is healing and unfamiliar products can cause more trouble than the itch.

When is itching a warning sign?

Call the clinic if itching comes with spreading redness, warmth, pus or discharge, an unpleasant smell, fever, or increasing pain, since those suggest infection. Also call if you develop widespread flaking, scaling or a rash beyond the treated area, which may indicate dermatitis or a reaction to a product rather than ordinary healing, or if the itch is severe and unrelieved past the first month.

Frequently asked questions

How long does the itching last?
Usually a few weeks, peaking around the second week as crusts clear. Occasional itching during regrowth over the following months is also normal.

Can I use anti-itch cream?
Not without asking. Many contain ingredients unsuitable for a healing scalp. We can recommend something appropriate.

Is itching a sign the grafts are growing?
It is a sign of healing rather than a reliable indicator of growth. Regrowth typically starts around three to four months, whether or not you itched.

Does the donor area itch too?
Often yes, and the same rules apply: no scratching, gentle washing, and tell us about anything that worsens.
